Policy Guide for Summer/After School Program:
1. Program Goals and Objectives:
The program's main goal is to provide a safe and enriching environment for participants during the summer and after school hours.
- The program aims to offer a variety of educational, recreational, and cultural activities to engage participants and promote their social, emotional, and physical development.
2. Eligibility Criteria:
- The program is open to students in K-5th grade who meet the age requirements.
Priority will be given to students from low-income families, and students who may be at risk of academic or social challenges.
3. Program Hours and Schedule:
- The program will run during the summer months from 7 am to 6 pm and after school hours on weekdays from 3 pm to 6 pm.
- The schedule will vary depending on the age of the participants and the specific activities planned for each day.
4. Staffing and Supervision:
- The program will be staffed by qualified employees and volunteers who have been properly screened and trained.
- There will be a designated program director who will oversee the daily operations and ensure the safety and well-being of participants.
5. Participant Behavior and Expectations:

Summer Program:

4. Responsibility for Personal Belongings:
- Participants are responsible for their personal belongings, including backpacks, supplies, and equipment. They should keep their belongings organized and secure to prevent loss or damage.
5. Safety and Well-being:
- Participants are expected to prioritize their safety and well-being by following safety instructions, staying within designated areas, and reporting any concerns or incidents to program staff promptly.
6. Respect for Diversity and Inclusion:
- Participants should respect and celebrate the diversity of their peers, including differences in backgrounds, cultures, and abilities. They should embrace inclusivity and show kindness and empathy towards others.
7. Communication and Collaboration:

LEEP Permission Form Waiver

I understand that personal injury can and may occur to my child, and I hereby authorize a LEEP representative to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.
I hereby release LEEP, its employees, agents and volunteers, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury (including death) that may be sustained by my child while participating in or traveling to and from this event.
The following is all of the insurance information, restrictions, allergy and medication information necessary for my child to receive appropriate medical care.
